GA-990fxa-ud3 Rev 4.0 - USB 3.0 problem after installing VIA driver

I went to the download site for rev 4.0 and noticed a VIA USB 3.0 driver I had never noticed before.  There have been some minor issues with devices plugged into the front panel USB ports so I decided to try to install it, thinking it was just your typical USB 3.0 driver.  Only, after installing it, all of the USB 3.0 ports (front panel and the 2 in the back) stopped working altogether.  I can't get it to recognize any device on those ports.  Troubleshooting doesn't recognize that there is any problem.  The rest of the USB ports on the back of the motherboard (non 3.0) are still working fine.  I tried uninstalling the VIA USB3.0 driver, but that didn't help, and when I tried reinstalling it, it now tells me it can't be installed because I don't have the hardware for it.  If that's true, why did it install the first time?

Prior to making any changes, when I was looking at my USB drivers in Device Manager, I am sure there was another entry listed at the bottom, but I cannot recall exactly what it was, and it is no longer there, apparently uninstalled by the VIA USB 3.0 driver process.

Scanning for new hardware doesn't help.   And now I can't find anything else to download that might resolve the problem, as far as Windows Update is concerned, everything is working fine.  The USB 3.0 ports have just become invisible to my OS.

Anyone have any ideas?  A link to a USB 3.0 driver that is appropriate to this board that might help? This is my girlfriend's computer, and this is embarrassing :(

I have checked the BIOS and I don't see anything wrong, everything under peripherals seems to be the defaults according to the manual (other than that it's configured as a UEFI installation) but then I'm not sure which specific option would correlate to just the USB 3.0 ports.

Yes, the usb3 controller disappears from time to time and to bring it back you have to pwr off the machine - live with that as long as you use this board. It shouldn't be too bad but after win or driver updates usb3 vanishes.
