Just wanted to chime in and say that I've been seeing the EXACT same issues as the original poster. In the process of getting my RMA with Newegg. Memory sticks, PCI-E card, integrated video, monitor, power supply are all good, so I'm ruling it the motherboard. Will update when I try the install.

Obviously it could be the motherboard that is causing the problems but it just as easilly could be the CPU, the monitor or the cable assuming that the GPU is working ok.
