I don't like that design with the fan all the way at the end. Is that the best way to design for cooling for really high end cards?

It's the standard reference design. There are other options and aftermarket coolers that can be used instead. Or there is liquid cooling

In my case it was drip, drip, drip "OH @#$%".

Water was dripping from the reservoir onto my HD5970, luckily it has a backplate and all is well

Scariest experience ever...
