Title: cmi8738/pci-sx sound card on GA945PLM giga =code12 error

 i used cmedia  pci sound card cmi8738/pci-sx and ude it on GA945PLM mother board
i have code 12 problem?
 i need applicable approach please ! :'(

It can't find I/O addresses or an IRQ.
