GV-N730D5-2GI: DVI-D Port Signal

Hello all,
I'm struggling to find out what the issue is for what I intend to setup with my PC and its displays.
Main objective: Extend desktop across two of the same monitors and clone the desktop display onto one television.
I have this PC (
http://www.microcenter.com/product/438579/M32AD-US026S_Desktop_Computer
) with this graphics card (
http://www.newegg.com/Product/Product.aspx?Item=N82E16814125775
) that has HDMI, DVI-D and VGA output ports.
I want to use these monitors (
http://www.tigerdirect.com/applications/SearchTools/item-details.asp?EdpNo=8297949&CatId=5469
) that have VGA and HDMI input ports.
I also want to use this television (
http://www.amazon.com/Samsung-UN32H5203-32-Inch-1080p-Smart/dp/B00K4UIO9K
) that has an HDMI input port.
[And just in case this information is also helpful, I use this power supply unit in the PC (
http://www.tigerdirect.com/applications/SearchTools/item-details.asp?EdpNo=7850412&CatId=1079
)]
Current Setup:
Right now I have the television connected through the HDMI port and two monitors connected through the DVI-D and VGA ports. According to its website (
http://www.gigabyte.us/products/product-page.aspx?pid=5113#sp
), I should be able to have 3 displays.
The TV connects through an HDMI to HDMI, one monitor connects via VGA to VGA and the other monitor physically connects to the DVI-D port through a DVI-D (male) to HDMI (female) video adapter (
http://www.microcenter.com/product/355991/HDMI_Female_to_DVI_Male_Adapter
) attached to the graphics card and an HDMI to HDMI cable to the monitor.
The video adapter connects exactly to the DVI-D port with matching 24+1 pins design.
The Issue:
The problem in my experience involves anything that connects to the DVI-D port. After I power on the computer the DVI-D monitor does in fact display the ASUS PC startup screen but then blacks out when the PC loads Windows 10.
At first I thought maybe it's the monitor and so I tried connecting the DVI-D port to my television in the same way I did to the monitor and I still don't see any differences or improvements.
I feel as if I should be able to achieve my objective without further additional purchases.
Any thoughts?
Thank you very much for your time.
